Application for the use of IT systems and IT services at the university (part 1)

If you do not have a university ID of the Münster university, then you can apply for a university ID here.

Important: This application must be completed by you personally!
It is not permitted for other people, such as university staff, to fill it in!
However, you may of course ask university staff to help you.

*Gender:		Please select your gender.
Title:		Please enter your academic title for correspondence purposes, e.g. “Prof.” or “Dr.”
*Complete given name (as in your passport):		Please provide your full given name as it appears on your ID card or passport. (We are obliged to record this data in accordance with § 172 TKG.) If you have more than one given name, you can later select in our IT portal which ones will be put into our systems.
*Complete surname (as in your passport):		Please provide your full surname as it appears on your ID card or passport. (We are obliged to record this data in accordance with § 172 TKG.) If you have more than one surname, you can later select in our IT portal which ones will be put into our systems.
Former surname:		If you had a university ID of the Münster university formerly and you have changed your surname in the mean time, please give the complete former surname.
*Date of birth (format YYYY-MM-DD):		Please enter your date of birth.
*Place of birth:		Please enter your place of birth and the country this place belongs to (today).

*Email:		Please enter your email address. You will be informed by email when your university ID is ready. This email will contain your university ID but not your password. So if you do not have an own email address yet you can ask an acquaintance or a colleague whether you may enter his/her address.

*Place of residence (with postal code):		Place of residence (with postal code, country dependent written before or after the place name), at most 100 characters
